Immunization Record Rules Agreement/Information Receipt Acknowledgement Parent's Release/Field Trip Consent/AV & K Waiver Food Program Application Child Care Data Collection Form (Subsidized Only) Emergency Medical Treatment Form Copy of Birth Certificate (Subsidized Only) Enrollment will be open to children ages 12 months and independently walking to 36 months. State Preschool serves children ages 3 and 4 (must be that age by December of that academic year).
